Care Planning Software For Older People & Dementia
With an ageing population, the need for an online care planning for providers of care and support to older people and those with dementia is increasingly important. New models of care and use of care management software are driving improvements in services and quality-of-life for service users
iplanit’s unique person-centred design promotes inclusion and control with a focus on outcomes amplifying ‘Think Local, Act Personal’, the Care Act and other transformation initiatives.
Service Users’ can have the ability to involve family-members, Social Workers and other members of their circle-of-care in the care planning and delivery cycle. This effective care management platform approach enhances cooperation and coordination, which in turn improves service quality and reduces the risk of isolation.
Real-time monitoring data allows providers to identify risks and coordinate proactive and preventative interventions, whilst providing clear evidence of quality to commissioners and regulators.
As care plans belong to the Service User, rather than the provider, electronic care planning means they remain consistent even when moving between services. The outcome-focused design of the iplanit software means that short-term, intermediate or rehabilitative services such as Reablement or physiotherapy are easily incorporated into overall packages of care.
Optional Modules featured below can be rolled out in addition to Core iplanit
The Location Management Module
The Location Module further enhances service delivery in Extra Care Housing and residential environments. Providers with multiple sites can plan location specific group events online using the iplanit system. Real time recording of notes and incidents that are specific to that service is also possible using the location module.
The Support Tasks Module
The Support Tasks Module allows providers to schedule and record regular or one-off activity such as personal care, meal preparation or medical appointments. This allows staff and management more time to concentrate on outcome-focused activities that improves overall quality-of-life for Service Users.
